Want to know more about transgender voice feminization and gender affirmation surgery? In this first episode in a new mini-series, Mr Chadwan Al Yaghchi, Consultant Surgeon, gives us insight into the surgical processes involved. He talks about pre-op, post-op, recovery time, what to expect in terms of end results, and the importance of good resources and working in conjunction with speech language therapists and other health professionals.

For patients, Mr Al Yaghachi covers the steps to take in finding a surgeon and your team. For therapists,  he has great suggestions for getting to know exactly what surgeons do.

Discussion & Reflection Question

    1. If you’re a therapist, who can you reach out to watch a surgery firsthand?
    2. If you’re a potential patient, have you thought through your options and expectations of how therapy and surgery work hand-in-hand?
    3. What resources and materials are you aware of that can help others – can you help address gaps?
    4. How can you work with people to ensure it’s an individualized experience?
    5. Who is your team? What wrap-around support and services do you need for holistic care?

Mr Chadwan Al Yaghchi is an innovative, experienced and respected consultant ear, nose and throat surgeon. He specializes in adult and pediatric laryngology. He treats many throat disorders, including airway stenosis, voice disorders, swallowing difficulties in adults and children and much more.

Mr Al Yaghchi also has a specialist interest in voice feminisation surgery, which was developed during his time working in close proximity to the Gender Identity Clinic in London. He went on to introduce a number of voice feminisation procedures to the UK. This included his own modification to the Wendler Glottoplasty technique, which has since become the preferred method for voice feminisation. Through his pioneering work in the field of gender affirmation surgery, Mr Al Yaghchi has been able to help hundreds of trans women to achieve a voice which more accurately reflects their gender identity.

Outside work, Mr Al Yaghchi’s main interest is cooking and he used to write a food blog. He would have been a chef in a different life.

“I got the privilege of being exposed to gender affirming surgery in early residency. I saw firsthand how much of a difference it makes to people’s lives… Everyone should know that this thing exists. I have clients that come to me, and it’s like, “I never knew there is such a thing”. … I think it is an essential part of gender transition. It’s a gender affirming surgery. It’s not a cosmetic procedure. It makes a huge difference to a person’s life”.

Some of the clients that I have worked with, they can get so far with the gender affirming voice work that we do, but they need that surgery to get them to that final stage. …. I understand the moment they open their mouth they’re misgendered, and how painful that is, and it just cuts down on who they can authentically be”.

Medical Speech-Language Pathologist and Founder of Fresh SLP.

Mattie Murrey-Tegels is the founder and SLP behind Fresh SLP and The Missing Link for SLPs Podcast. She’s been “in-the-trenches” as a medical SLP around the world for over 25 years and now an Assistant Professor for 3 years. She is thrilled to be adding this dream of a podcast because paying her experiences forward is something she is very passionate about. If you ask her patients and students, one thing they will remember is how much she loves her job! She may not look like it but she is a huge introvert and when she is not actively working as an SLP, she is almost always reading, writing (writing over 1,000,000 words a year), or listening to amazing Chicago Blues bands. She also loves being outdoors and definitely enjoys soaking up the sun at her home in Minnesota, where warm and sunny days can be limited. She’s ridden motorcycles for many years, raced sled dogs, hiked huge mountains yet she cherishes the quiet moments of climbing into a hammock to nap or timeless conversations with friends and family.

The Missing Link for SLPs podcast and Fresh SLP is her legacy, giving back to a career that has so richly rewarded her.
